DAS 140 and DAS 142 apprentice forms
Apprentice paperwork follows the craft and the project schedule. Award information and a dispatch request serve different purposes, so keeping a copy of the award form alone does not complete the dispatch task.

Check the craft and apprenticeship obligation
DIR states that public works contracts valued at $30,000 or more carry an obligation to hire apprentices unless the craft or trade does not require them. Review the applicable craft and project circumstances with the apprenticeship program before scheduling workers.
Route award information to the right committees
DAS 140 gives contract award information for each craft. A contractor approved to train sends it to its apprenticeship committee; one not approved sends it to all committees that can supply apprentices to the site. The form says not to send it to DAS and explains that it is not a dispatch request.

Build the award form from the job file
Use the contract and project records to gather the contractor details, awarding agency, execution and start dates, estimated journeyworker and apprentice hours, and intended apprenticeship programs. The Labor Commissioner’s manual describes the award notice timing as within ten days of contract execution and no later than the first day workers are on the job.

Request dispatch ahead of the apprentice start
DAS 142 is the separate written dispatch request. DIR calls for at least three business days before apprentices are needed; the form specifies 72 hours excluding weekends and holidays. Retain evidence of sending the request and the response with the planned work dates.

Track apprentice hours and training contributions
DIR describes an apprentice-to-journeyworker ratio of one hour to five hours. Keep time records by craft so the project can be reviewed. Training contributions follow the amount in the prevailing wage rate and go to the applicable committee or CAC; the DAS forms page identifies CAC-2 for reporting those contributions.

Connect apprentice records with project payroll
Tidwell organizes hours and payroll support for certified reporting. Bring committee correspondence and project instructions so missing records can be identified.
